New Client Intake Forms
Important information that helps us create a treatment plan for your child.
Parents or Guardians of new clients can fill out the forms and submit them digitally. Or, if you would prefer to print and fill them out, you can bring them with you for your child's first visit. Scroll down for printable PDF forms for both children and adults.

Almost done! If you have printed and filled out the PDF Consent Forms from Step 2: The Consent To Treatment Form and The Consent to Electronic Transmission Form, simply bring them to our office.

All Done! Once you complete the above steps, you are all set for your child's first appointment.
